Overview

In 2022, I was deployed to a power station on the outskirts of Baghdad, Iraq. This was a major unexpected forced outage that happened just as the Iraqi national power grid was braced for the summer heat run with all the air conditioning load. The timing could not have been more unfortunate for this customer. Furthering the pain factor, the unit was virtually band-new and had just come out of new unit commissioning. The frame 324 stator fell victim to a known design deficiency in this specific design. Getting all of the parts, tooling, and supplies to this extremely difficult location was a major effort.

The Challenge

A unit suffered an in-service failure due to an open circuit condition on one, followed by multiple locations of high cycle resonance-related mechanical issues that caused the strands to break. The open circuit event that ensued caused a catastrophic loss in the stator, ultimately requiring a full stator rewind.

Investigation and Discovery

I participated in the detailed Root Cause Analysis (RCA) of the failed stator. Working methodically through the damaged end-windings and collector ring area, we performed a comprehensive inspection of the phase connection rings and associated supports. The investigation revealed a critical design and installation vulnerability:

Once the resonant frequency of the failing windings reached the driving frequency of the system this fundamental excitation mode quickly drove the windings to failure. This resonance-induced failure mechanism explained the sudden and severe nature of the outage.

Solution Development

With the root cause clearly established, I provided the customer with a detailed RCA report, including photographic evidence, dimensional measurements, and specific recommendations for improved blocking and support of the end connections. Another vendor was selected to perform the full stator rewind. They visited the site, accurately measured the winding geometries, manufactured a complete set of replacement windings, and successfully executed the rewind on location. My role focused on the critical RCA phase and technical oversight during the early stages of the repair planning.

Results and Customer Impact

The RCA provided the customer with a clear understanding of the failure mechanism and actionable solutions to prevent similar issues across their fleet of steam turbine generators. The full stator rewind was completed successfully, returning the generator to reliable operation. The detailed documentation and lessons learned from this event have since been incorporated into the plant’s maintenance and inspection strategies for their GE hydrogen-cooled fleet.
